Cost effectiveness analyses from developed countries cannot be applied to developing countries. Successful VNS therapy does reduce the financial burden of the device, however direct cost savings are difficult to achieve. There is acceptable QALY gain with VNS therapy in developing countries, with encouraging cost per QALY. Appropriate instruments for deducing utility values for children with epilepsy are needed. Abstract: Purpose: To evaluate clinical outcomes, quality-adjusted life years (QALY), cost effectiveness and cost utility associated with VNS therapy in children with refractory epilepsy in a developing country. Methods: Retrospective review of all children who underwent VNS implantation at King Abdullah University Hospital and Jordan University Hospital in Jordan. Results: Twenty eight patients (16 males) had implantation of the VNS therapy system between the years 2007 and 2011. Mean age at implantation was 9.4 years. Mean duration of epilepsy prior to implantation was 6.5 years. The most common seizure type was generalized tonic clonic seizures. Fifteen patients showed a 50% or more reduction in seizure frequency. There was a significant reduction in total number of seizures ( p = 0.002) and emergency room (ER) visits ( p = 0.042) after VNS therapy. Atonic seizures were more likely to respond than generalized tonic clonic seizures, p = 0.034. Direct hospital costs prior to VNS implantation were analyzed in relation to ER visits and intensive care unit (ICU) admissions. Cost savings per patient did reduce the financial burden of the device by about 30%. There was a QALY gain per lifetime of 3.78 years for children and 1 year for adolescents. Conclusion: Response to VNS implantation in Jordan was favorable and similar to what has been previously reported. QALY gain and cost per QALY analysis were encouraging. Cost savings were related to reduction in seizure severity. In circumstances of limited resources as in developing countries, targeting patients with frequent utilization of health services would improve cost effectiveness.

The effects of heart rate-triggered VNS-pulses on seizure duration were evaluated. The novel VNS-device detected seizures with high sensitivity but low specificity. Responsive VNS-stimulation significantly reduced seizure duration. Abstract: Purpose: A novel vagus nerve stimulation (VNS) device was recently approved in Europe which rapidly detects increases in heart rates (HR) and applies an additional stimulus if HR-increases exceed a given threshold. The effects of HR-triggered VNS-pulses on seizures were not reported yet under controlled conditions. Here, we quantified the effects of HR-triggered VNS-pulses on the seizure duration in one patient. Methods: The novel VNS device was implanted in a 29-year old man with refractory epilepsy. After implantation, the patient underwent video-EEG telemetry for 68 h with no changes in anticonvulsant drugs. On the first day the patient only received sham-stimulation. During the following 46 h HR-related VNS-stimulation was set to 2 mA. Seizure duration was determined based on clinical signs. Results: Twelve stereotypical seizures were recorded (six during sham- and six during the active stimulation). The VNS device recognised a total of 139 events as a seizure and correctly identified 11 seizures. The HR-triggered VNS-stimulation significantly reduced the total seizure duration from 33.2 ± 4.8 s to 26.5 ± 5 s and the remaining seizure duration after the onset of the extra-stimulation from 27.8 ± 4.3 s to 16.2 ± 3.2 s. With the given configuration in this patient, sensitivity and specificity of HR-based seizure-detection amounted to 92% and 13.5%, respectively. Conclusions: This case illustrates that VNS-stimulation in response to seizure-related HR-increases is able to significantly reduce seizure duration. Despite the limitations of our case, its promising results should prompt larger studies to confirm the clinical benefit of this novel device.

✏Modulation of Heart Rate by Temporally Patterned Vagus Nerve Stimulation in the Anesthetized Dog Book Summary : Abstract: Despite current knowledge of the myriad physiological effects of vagus nerve stimulation (VNS) in various mammalian species (including humans), the impact of varying stimulation parameters on nerve recruitment and physiological responses is not well understood. We investigated nerve recruitment, cardiovascular responses, and skeletal muscle responses to different temporal patterns of VNS across 39 combinations of stimulation amplitude, frequency, and number of pulses per burst. Anesthetized dogs were implanted with stimulating and recording cuff electrodes around the cervical vagus nerve, whereas laryngeal electromyogram (EMG) and heart rate were recorded. In seven of eight dogs, VNS‐evoked bradycardia (defined as ≥10% decrease in heart rate) was achieved by applying stimuli at amplitudes equal to or greater than the threshold for activating slow B‐fibers. Temporally patterned VNS (minimum 5 pulses per burst) was sufficient to elicit bradycardia while reducing the concomitant activation of laryngeal muscles by more than 50%. Temporal patterns of VNS can be used to modulate heart rate while minimizing laryngeal motor fiber activation, and this is a novel approach to reduce the side effects produced by VNS. Abstract : Vagus nerve stimulation‐evoked bradycardia has been identified as an important biomarker for effective treatment of heart failure. We have shown in this preclinical study that very brief bursts of electrical pulses applied at the threshold for recruiting slow B‐fibers can effectively reduce heart rate while minimizing the activation of laryngeal muscles. We predict this novel pattern of nerve stimulation will improve the therapeutic efficacy of vagus nerve stimulation in heart failure patients.

VNS has been shown to decrease the amount of inflammatory cytokines (TNF-alpha, IL-6, IL-8 and IFN-gamma) and subsequently reduce the production of C-Reactive Protein. Furthermore, VNS also leads to decreased pain perception. There has been growing interest in the application of vagus nerve stimulation in the treatment of a constellation of medical issues that involve a perturbation of normal plasma cytokine profile. More recently, a phase II clinical trial for subjects with rheumatoid arthritis has shown that there was a significant reduction in pain and a decrease in C-Reactive Protein (CRP) in patients who received a surgically implanted VNS device that provides a maximum of 5 minutes of stimulation daily. Disruptions in the innate immune system have been shown to be one of the mechanisms that put patients at risk for developing Post-Traumatic Stress Disorder. ✏The Effects of Vagus Nerve Stimulation on Extinction of Conditioned Fear Book Summary : Extinction of conditioned fear is the basis for exposure-based therapies that are used to treat disorders including posttraumatic stress disorder (PTSD), obsessive compulsive disorder (OCD), and specific phobias. Though exposure-based therapies are the best evidence-based treatment for anxiety-related disorders, nonresponse and dropout rates are still as high as fifty percent. Failure during or after treatment could stem from generalized extinction impairments seen in patients with PTSD, OCD, and specific phobias. Adjuvant treatments that could enhance extinction of conditioned fear, and improve treatment tolerability would be useful in improving treatment outcomes for patients with anxiety-related disorders. Vagus nerve stimulation (VNS) has been safely used in tens of thousands of patients for epilepsy and treatment resistant depression since FDA approval in 1997. VNS enhances memory consolidation in rats and in humans, and can also enhance extinction of conditioned fear in healthy rats. Here, we expand findings regarding VNS and fear extinction. We found that VNS can enhance extinction of fear in a rat model of PTSD that is resistant to extinction, as well as reduce the incidence of relapse and reverse PTSD-like symptoms including avoidance and heightened anxiety more than one week following treatment. We also found that VNS can generate more broad extinction learning by promoting generalization of extinction of conditioned fear. Though VNS is anxiolytic, our findings also indicate that the mechanism by which VNS enhances extinction of conditioned fear is not through a reduction in anxiety. Taken together, our findings indicate that VNS could be a useful adjunct for exposure-based therapies to improve treatment outcomes by enhancing extinction, and improve tolerability by reducing anxiety.

✏BETTER WIRING FOR A FAULTY METABOLISM VAGUS NERVE STIMULATION MODULATES GLYCAEMIA VIA PANCREATIC HORMONES Book Summary : IntroductionDespite advancements in pharmacotherapies to modulate glycaemia in type 2 diabetes, glucose control remains suboptimal in a large proportion of patients. This project aims to explore the feasibility of an electroceutical (neuromodulation) approach to reducing glycaemic variation. The vagus nerve is an important modulator of pancreatic endocrine function and in this study we evaluated the effect of electrical stimulation frequency of the vagus on glycaemic control. MethodsNon-diabetic, male, Sprague-Dawley rats (n = 6) were anaesthetised and the vagus nerve implanted with a vagus nerve electrode array. The femoral veins were cannulated and a glucose dose (500 mg/kg) administered i.v. or through a gastric catheter. Vagus nerve stimulation (15 or 4000 Hz) was applied over 60 minutes following i.v. glucose, and serial blood samples taken for glucose and hormone analysis. No-stimulation was used as a crossover control sequence. ResultsLow frequency stimulation produced a significant increase in glycaemia, with a net effect of +235u00b159 mMu00b7min over 60 minutes above the control glucose dose (p = 0.028). This was accompanied by a decrease in insulin (-25u00b115 ng/mlu00b7min) but an increase in glucagon (+1.25u00b10.44 ng/mlu00b7min) above control. In contrast, high-frequency stimulation was associated with a smaller change in the glycaemic response (+62u00b135 mMu00b7min, p = 0.06).ConclusionIn summary, vagus nerve stimulation modulates glycaemia by effecting insulin and glucagon secretion. Stimulation frequency appears to modulate the glycaemic control and should be studied further. We anticipate that electrical stimulation could be applied as a modality to help negate glycaemic variability in type 2 diabetes.

✏NON INVASIVE VAGUS NERVE STIMULATION NVNS REDUCES PERI INFARCT DEPOLARIZATIONS AND IMPROVES STROKE OUTCOME IN RATS Book Summary : Vagus nerve stimulation (VNS) has been shown to reduce cortical spreading depression (CSD) (Chen et al., 2016) and improve stroke outcome (Ay et al., 2009). Peri-infarct depolarizations (PIDs), i.e. the pathophysiological equivalent to CSD in acute stroke, strongly contribute to infarct growth in the penumbra by exacerbating local blood flow reduction, inducing metabolic stress, and by triggering excitotoxicity and neuroinflammation (Rakers and Petzold, 2016). In anesthetized Wistar rats (9-11w, males), we here aimed to investigate the therapeutic potential of non-invasive (n)VNS on PIDs during middle cerebral artery occlusion (MCAO). Controlling regional cerebral blood flow (rCBF) by laser speckle contrast imaging, we induced permanent MCAO and analyzed PIDs after transcutaneous nVNS, as well as after invasive VNS (iVNS), by electrophysiology for 4 hours after filament occlusion. Concomitantly, we investigated the effect of nVNS during transient MCAO on motor function and infarct volume.Both VNS approaches (nVNS and iVNS) showed a significant decrease of PID frequency during permanent MCAO. During transient MCAO, nVNS significantly reduced the cortical infarct volume after 72 hours compared to control animals. Accordingly, behavioral data appeared to indicate an improvement of motor function by nVNS. In conclusion, VNS in acute ischemia improves stroke outcome by reducing the frequency of PIDs, implicating nVNS as a potent non-invasive method to mitigate neurological damage after stroke.

✏Extinction Learning Paired with Vagus Nerve Stimulation Enhances Behavioral Outcomes and Drives Plasticity in Extinction Networks Book Summary : Extinction learning is integral to the treatment of substance use disorder (SUD), and anxiety and trauma related disorders such as phobia, generalized anxiety, and post-traumatic stress disorder (PTSD). These disorders feature maladaptive responses to conditioned stimuli. In SUD, patients exposed to drug-associated cues or environments experience anxiety, craving, and renewal of drug-seeking behavior. In phobia and PTSD, patients develop fearful responses to otherwise harmless stimuli, and when exposed to these conditioned stimuli experience disruptive levels of fear, avoidance, and anxiety. Unfortunately, extinction learning is frequently impaired in these patient populations, making extinction-based cognitive behavioral exposure therapy difficult. Several interventions have been applied to increase the efficacy of exposure therapy including the use of drugs such as antidepressants, anxiolytics, or nootropics; other types of cognitive behavioral therapy such as mindfulness or coping strategies; and deep brain stimulation among others, each with varying degrees of success. Vagus nerve stimulation (VNS) is FDA-approved for the treatment of epilepsy and treatmentresistant depression, and has been found to enhance memory consolidation in rats and in humans and to drive other types of neuroplasticity. The work of this dissertation investigates the use of VNS to enhance extinction learning in a model of conditioned fear and in a model of drugtaking. In Chapter 2 I describe experiments that examined the effects of VNS on the extinction of an auditory fear memory. In these experiments we found lower rates of conditioned fear in animals that were given VNS during extinction. When we investigated connectivity between the infralimbic prefrontal cortex (IL) and the basolateral amygdala (BLA), a pathway involved in extinction learning, we found that the conditions for the induction of plasticity were changed in VNS animals compared to sham-stimulated animals. In Chapters 3 and 4 I describe experiments in which we applied VNS during extinction from drug-seeking, and found reduced renewal of drug-seeking during cued- and contextual renewal. As in our model of fear conditioning, VNS animals extinguished from drug-seeking also had altered IL-BLA connectivity. These results provide preclinical evidence for VNS as an effective adjunct to extinction training. Because of the advantages treatment with electrical stimulation offers over other methods, VNS paired with extinction should be further investigated for use in the clinical setting
