✏Bronchopulmonary Dysplasia Book Summary : Bronchopulmonary dysplasia (BPD) as a chronic lung disease affecting preterm infants has been recognized for more than 45 years. However, little progress has been made in the prevention and treatment of the disease. BPD continues to be a major morbidity affecting preterm infants. Studies in pre-adolescent children demonstrate that the abnormal lung function from BPD persists well into childhood. Infants with BPD also have an increased risk for adverse neurodevelopmental outcome. Thus BPD imposes a significant burden of adverse health outcomes in preterm infants. Neonatal pulse oximetry study is an objective method to guide oxygen therapy to prevent life threatening events, optimize growth and reducing pulmonary hypertension.Objective: To identify variations in methodology in conducting pulse oximetry studies and its potential effect on the study results, in infants with NCLD admitted to NICU or in the communityMethods: A retrospective review of 144 pulse oximetry studies(March 2016u2013March 2018).Clinical data was collected from patient notes, community nursing database and archived oximetry reports generated from Nellcor pulse oximeter. Results: Total of 144 studies, 53 studies conducted in NICU (36%) and 91 studies (64%) conducted in community. Mean duration of recording of studies done on NICU was 15 hours and 6 minutes and the one done in community was 11 hours and 12 minutes. In more than half of the studies (51%) time of interference was not documented. Average of percentage of interference on the studies done in community was 9% compared to17% on the studies done in NICU. When the duration of the study more than 12 hours, odds of getting abnormal study results were 3.6 (CI 1.9-6.7 P0.0001) and odds of having interference 8% of the recording, were 2.7(C.I. 1.9-48 P=0.006)Conclusions: If the duration of pulse oximetry study is more than 12 hours, it is more likely to be abnormal and to have a significant percentage of interference. There is variation in methodology of conducting pulse oximetry studies in NICU compared to in community for infants with NCLD.
