✏Transsphenoidal Surgery for Pituitary Tumours Frequency and Predictors of Delayed Hyponatraemia and Their Relationship to Early Readmission Book Summary : Abstract : Objective: A major cause of readmission after transsphenoidal surgery (TSS) is delayed hyponatraemia. The purpose of this study was to identify predictors of hyponatraemia one week post surgery and predictors of 30-day readmissions for hyponatraemia. Design: A retrospective cohort study including patients who had TSS performed for pituitary lesions. Method: The risk of readmission for hyponatraemia was assessed in consecutive patients between January 2008 and March 2016. The risk of hyponatraemia one week post surgery was assessed in patients admitted for TSS between July 2011 and March 2016. Results: Of all included patients, 56/522 (10.7%) were readmitted within 30 days. Hyponatraemia was found in 14/56 (25%) of 30-day readmissions. We did not identify any predictive variable for hyponatraemia on readmission. The number of patients with hyponatraemia on the seventh post-operative day was 26/314 (8.3%). The risk of hyponatraemia one week post surgery was increased by an odds ratio of 2.40 (95% CI: 1.06–5.40) in patients with a tumour abutting the optic chiasm and by an odds ratio of 1.16 (1.04–1.31) per mmol/L decrease in sodium levels on the first post-operative day. Conclusions: Hyponatraemia occurred in 25% of readmissions; however, we did not identify any predictive variable for readmission with hyponatraemia. One week post surgery, 8.9% had hyponatraemia. Tumours pressing on the optic chiasm as well as a fall in sodium levels on the first post-operative day were associated with an increased risk of hyponatraemia one week post surgery. It was therefore not totally surprising when Professor A. N. Konovalov proposed this topic for discussion at the Fifth Annual Stonwin Medical Conference. Bringing the leading figures in this area to New York for a roundtable conference was an exciting challenge. Professor Russel H. Patterson, Jr., generously consented to be our guest of honor. The conference was held at the Winston Estate on July 13-14, 1987, and met our every expectation for a vigorous exchange of individual experiences and more importantly for a dialogue directed toward present and future expectations in the surgery of this region. In addition, the discussions at that small dinner at the Harvard Club of New York led to the creation of an exchange program for neurosurgical residents, fellows, and faculty members of the N. N. Burdenko Neurosurgical Institute, Academy of Medical Science, and the Department of Neurosurgery of the New York Neurological Institute, which was planned for the spring and summer of 1988. The Stonwin Conference and the exchange agreement and program fostered by the Harry Winston Medical Foundation, Inc., represent the fulfillment of goals con ceived by Harry Winston and promulgated by his sons Ronald and Bruce. The meeting was organized to commemorate the description of acromegaly by the French physician Pierre Marie, in 1886. The members of the Scientific Committee spent many hours assisting us in ensuring an outstanding meeting. The support of Serono Symposia, USA in all phases of the planning and execution of the meeting was sincerely appreciated and was highly professional. Special recognition roust be extended to Professor Roger Guillemin of the Salk Institute, whosp interest in medical history led him to devote a great deal of time and personal expense in obtaining information about the life of Pierre Marie. Dr. GuilIemin's presentation on the life and times of Marie was an extraordinary overview of the cultural, social, and scientific backgrounds in which Marie came to describe the disease, acromegaly. Dr. Guillemin' s findings, which were presented at the main banquet in a splendid audiovisual presentation, were clearly the highlight of the meeting and will long be remembered by the attendees. We were also honored by the presence of Dr. Martine Pierre Marie Granier, the great-granddaughter of Pierre Harie. In her address at the banquet, Dr. Granier provided several delightful and intimate vignettes concerning Pierre Marie, using old family records and photographs. The Scientific Committee wishes to extend its gratitude to Dr. Granier for adding immeasurably to the historical atmosphere of the meeting.
