✏QT Prolongation Torsade de Pointes Myocardial Ischemia From Coronary Vasospasm and Headache Medications Part 1 Review of Serotonergic Cardiac Adverse Events With a Triptan Case Book Summary : Abstract : Serotonin (5‐hydroxytryptamine)1 B /1 D agonists are vasoconstrictors that can affect coronary and cerebral arteries. Retrosternal chest, arm, and jaw pain following triptan use is generally attributed to "triptan sensations" and dismissed as noncardiac. However, triptans narrow normal coronary arteries and occasionally trigger vasospasm. They are contraindicated in atherosclerotic vascular disease. Part 1 of this review examines the relationship of medications used in migraine with the likelihood of causing vasospasm or vasoconstriction, and the triggering of cardiac arrhythmias. We report an illustrative case of polymorphic ventricular tachyarrhythmia, electrocardiogram changes consistent with cardiac ischemia, and acquired corrected QT interval lengthening following oral sumatriptan in a 53‐year‐old migraineur without risk factors for coronary artery disease (CAD). Extensive evaluation revealed insignificant single coronary vessel atherosclerosis and coronary artery vasospasm on ergonovine challenge. The report highlights the hidden risk that may underlie a "triptan sensation" and the possible association of the vasospastic features of Raynaud's phenomenon, migraine headaches, and coronary vasospasm. Part 1 discusses the risks for Torsade de Pointes, vasospasm, and ischemia, with a review and discussion of case reports of triptan‐associated cardiovascular events in migraineurs with and without CAD risk factors or documented CAD; of the epidemiology and studies of triptans, vasospasm, and cardiovascular morbidity; and of the relationship of variant angina, migraine, and vasospastic disease. In the second part of this review, headache medications and their propensity for corrected QT prolongation will be summarized.

✏Erythromycin QTc Interval Prolongation and Torsade de Pointes Case Reports Major Risk Factors and Illness Severity Book Summary : Objectives: Erythromycin is a macrolide antibiotic that is widely used for various infections of the upper respiratory tract, skin, and soft tissue. Similar to other macrolides (clarithromycin, azithromycin), erythromycin has been linked to QTc interval prolongation and torsade de pointes ( TdP ) arrhythmia. We sought to identify factors that link to erythromycin-induced/associated QTc interval prolongation and TdP . Methods and Results: In a critical evaluation of case reports, we found 29 cases: 22 women and 7 men (age range 18–95 years). With both oral and intravenous erythromycin administration, there was no significant relationship between dose and QTc interval duration in these cases. Notably, all patients had severe illness. Other risk factors included female sex, older age, presence of heart disease, concomitant administration of either other QTc prolonging drugs or agents that were substrates for or inhibitors of CYP3A4. Most patients had at least two risk factors. Conclusions: On the basis of case report evaluation, we believe that major risk factors for erythromycin-associated TdP are female sex, heart disease and old age, particularly against a background of severe illness. Coadministration of erythromycin with other drugs that inhibit or are metabolized by CYP3A4 or with QTc prolonging drugs should be avoided in this setting.

The most popular method to calculate it is the Bazett formula, calculating the corrected QT interval: QTc. When the QTc interval is greater than 500 ms, the patient will be at risk of developing torsades de pointes (TdP).We present the clinical case of a 53-year-old women who suffers from TdP after receiving Olanzapine 10mg in the context of a psychotic episode.Methods:We propose to carry out a bibliographic review on the risk factors of presenting a prolongation of the QT interval and TdP, focusing on the psychotropic drugs and on the therapeutic alternativesResults:The risk factors for prolongation of the QT interval are multiple. When the risk of prolongation of the QT interval induced by drugs is considered high, a basal ECG should be performed as well as when the added drug reaches steady state. Aripiprazole stands out among the therapeutic alternatives in the group of antipsychotics, considering the antipsychotic that has less risk.Conclusions:The use of psychotropic drugs that affect myocardial repolarization cannot be entirely avoided. Nonetheless, every attempt should be made to avoid the combined use of multiple drugs that can prolong the QTc interval and elevate the risk of TdP.
