✏MENTAL PRACTICE FOR CHRONIC PAIN IN PEOPLE WITH SPINAL CORD INJURY Book Summary : Background and aimsThe management of chronic pain after spinal cord injury (SCI) is very challenging and there is a lack of evidence on the impact of both pharmacological and non-pharmacological treatments. Mental practice (MP) can be used as an adjunct treatment for motor recovery as well as pain relief. Until now, the effect of MP on pain in SCI patients remains unclear. In this systematic review, we aim to identify therapeutic benefits of MP for both pain management and motor function recovery in SCI people.MethodsFrom the 9924 non-duplicate titles of potential relevance, 19 (2 RCT, 9 quasi-RCT, 8 observational) studies were selected for final analysis and were rated using standardized critical appraisal assessment forms from the Joanna Briggs Institute. Because of significant heterogeneity in the included studies, we summarized intervention effects on outcomes of interest descriptively. ResultsThe included studies involved 307 patients. Results showed mainly improvement in motor function and pain, although some reported no effect or increase in pain. Training modalities of MP were heterogeneous, nevertheless most of the studies used shorter sessions (8 to 20 minutes) for pain management, and 30 to 60 minutes sessions for motor improvement. Neurophysiological data demonstrate reorganizations in brain activity, which could be corrected by MP.ConclusionsHigh heterogeneity in SCI population, interventions and outcomes measured, makes it hard to judge about therapeutic effects and best MP protocol, especially in SCI with neuropathic pain. Further clinical trials evaluating MP as adjunct therapy for pain in SCI patients are warranted.

✏EXPERIENCES OF PAIN RELIEVING DRUGS IN INDIVIDUALS WITH SPINAL CORD INJURY AND NEUROPATHIC PAIN A QUALITATIVE STUDY Book Summary : Background and aims: Neuropathic pain (NP) is described as one of the most challenging conditions for individuals with spinal cord injury (SCI) and is primarily managed pharmacologically. However, knowledge is lacking on the patientu2019s experiences of using these drugs. This study aimed to explore patientu2019s expectations, experiences and desires with drugs prescribed for SCI NP.Methods: 18 informants with SCI and NP were interviewed in focus groups consisting of 4-5 participants in four separate sessions. The informants originated from 6 large and small cities in southern Sweden. An emergent design was used using an interview guide containing 6 open questions. The interviews were transcribed verbatim and data was analyzed according to qualitative content analysis.Results: Initially expectations of drugs giving pain relief were described as rather high but these decreased over time due to negative experiences. Cognitive side-effects and/or insufficient pain-relief decreased further expectations and willingness to try new drugs. Still the informants described a remaining hope for a truly effective drug without side-effects. Since cognitive side-effects were described to impair relations and ability to work/study the informants described having to choose pain for cognitive clarity.Competence in SCI pain management and a functional dialogue with the treating physician, improved accessibility to health care, and an individually tailored treatment plan and followu2013up of prescribed medication were factors desired by the informants. Further, peer support and alternatives to a pharmacological approach were highlighted. Conclusions: The need for efficacious drugs, improved inter-personal health care relations and a patient-centred approach emerged.

✏01 ASSESSMENT OF CENTRAL SENSITIZATION IN NEUROPATHIC PAIN FOLLOWING SPINAL CORD INJURY Book Summary : Background aims:Causes of chronic pain are postulated to lie in abnormal pain modulation apparent in many diseases. The nociceptive system is balanced with certain anti- and pro-nociceptive mechanisms. Former includes the spinal inhibitory network assessed by conditioned pain modulation. Latter is due to a sensitization along the nociceptive neuraxis leading to ectopic excitability, contributing to hyperalgesia and allodynia. Temporal summation of pain (TSP) reflects this central sensitization.Methods:This study assesses pain modulation during tonic noxious heat application in three groups: Thoracic spinal cord injury (SCI) patients with and without neuropathic pain (NP), and matched healthy controls. The assessment was performed at the volar forearm in order to investigate a sensory intact area. A thermode was heated to 45u00b0C, and the participant was instructed to rate the pain on a numeric rating scale. Subsequently, they had to keep the pain level constant for two minutes by adjusting the temperature of the thermode (participant-controlled temperature, PCT). Clicking the mouse buttons increased or decreased the temperature, respectively.Results:The binary analysis of TSP occurrence highlighted that NP patients show the highest percentage of TSP occurrences (NP: 88%, nonNP: 63.6%, HC: 80%). Statistical analysis revealed a significant increase of the TSP magnitude and the TSP area under curve between the NP and nonNP SCI group.Conclusions:Increased temporal summation of pain u2013 acting as a proxy for central sensitization - was found in SCI patients with chronic NP. The method of tonic heat profiles enables a continuous measure allowing the analysis of adaptation and the TSP processes.

✏HIGH FREQUENCY SPINAL CORD STIMULATION AT 10 KHZ 10 KHZ SCS FOR THE TREATMENT OF CHRONIC NEUROPATHIC PAIN RESULTING FROM SPINAL CORD INJURY Book Summary : Background/aimsThere is not sufficient evidence supporting the use of spinal cord stimulation (SCS) in spinal cord injury (SCI)1. Retrospective studies showed promising results in SCI patients treated with high-frequency SCS at 10 kHz (10 kHz SCS)2,3. This prospective, feasibility study aims to evaluate the safety and effectiveness of 10 kHz SCS for chronic neuropathic pain in SCI patients.MethodsSubjects with chronic, neuropathic pain of u22655 cm (visual analog scale, VAS) in the pain area directly related to SCI were enrolled. Subjects were trialed with 10 kHz SCS. Subjects with a successful trial (u226550% pain relief) were implanted with a Senza system (Nevro Corp., Redwood City, CA, USA). Safety and effectiveness endpoints were captured up to 12 months post-implant. ResultsTwelve out of the 18 enrolled subjects underwent a trial implant; scar tissue precluded lead placement in 2 subjects and 6 out of the remaining 10 had a successful trial.Baseline mean (u00b1SD) pain scores of 9.0u00b10.9 cm (N=5) improved to 2.3u00b11.7 cm (N=5) at the end of trial. Pain scores remained improved at 3.1u00b11.6 cm (N=5), 4.8u00b14.0 cm (N=5), 5.1u00b13.3 cm (N=5), 1.7u00b10.6 cm (N=4) and 3.2u00b11.4 cm (N=3) at 1, 3, 6 and 12 month post-implant follow-up. Two subjects were reclassified from ASIA A at baseline to ASIA B at their 3, 6 and 12 month visits due to improved sensory function.ConclusionThis study provides promising preliminary results using 10 kHz SCS in SCI patients to alleviate chronic pain with a potential benefit of improved sensory function.
