There seem to have been two main reasons why the practice came under debate. On the one hand, some people felt that, when palliative medicine had advanced and methods to control symptoms had improved, it was no longer justified to sedate the patients in a manner that had often been done in the past. The system of 1 terminal sedation had turned into ‘euthanasia in disguise’ or ‘slow euthanasia’. On the other hand, there were people sympathetic to the recently established Dutch system of euthanasia, people who agreed that terminal sedation was euthanasia in disguise, but who felt that, if it is not objectionable to sedate dying patients at their request, then why should it not be permitted for doctors to kill dying patients at 2 request? ✏National Audit Safety of Deep Sedation Performed by Sedation Practitioners Book Summary : Background and Goal of studyIn the Netherlands, moderate-to-deep sedation is performed by specially trained sedation practitioners (SPS) under indirect supervision of an anesthesiologist. To evaluate safety of this practice a national registry of sedation related adverse events (AE) was established. Since the primary risk of sedation is airway related, we analysed all respiratory AEs from this registry.Materials and methodsThe prospective registry included all adult procedural sedation and analgesia cases in 20 Dutch academic and district general hospitals, from the 1st of February 2015 - 1st of December 2015. The EC stated that their approval of this study was not required. Data was collected with a modified world SIVA International Sedation Taskforce adverse event reporting tool1 and are presented as descriptive statistics.Results and DiscussionThe registry contains 7004 patients, 20% ASA-1; 61% ASA-2; 18% ASA-3 and 1% ASA-4, with a median age of 63 (IQR 49-71) that underwent 1617 oesophagogastrodoudenoscopies, 1050 endoscopic retrograde cholangiopancreaticographies, 1649 colonoscopies, 268 lung and 1120 cardiological interventions, 96 interventional radiology and 1400 other procedures. From the total of 974 (14%) AEs, 692 (10%) were respiratory related. There were 566 (8%) airway obstructions which needed an airway maneuver (i.e. head tilt, chin lift), 9 (0,1%) of these needed advanced airway management (i.e. intubation and mechanical ventilation). There were 409 (5,8%) desaturations which were all managed with increasing the inspired fraction of oxygen, an airway maneuver in 270 (3,9%), tactile stimulation in 33 (0,5%), a nasal pharyngeal airway or guedel in 84 (1,1%), mask ventilation in 70 (1%), CPAP in 4 and advanced airway management in 6 (0,1%) cases. A prolonged apnea occurred in 7 (0,1%) patients. Of the 24 patients that vomited, 5 (0,07%) aspirated, none of them developed severe aspiration pneumonia. No patients developed major sedation related complications or died of a sedation related AE.ConclusionPSA performed by trained SPS in several Dutch hospitals is associated with low AE and complication rates as reported in literature.2 On-going evaluation of care processes is required to ensure further performance improvement.References1. ✏Pediatric Procedural Sedation Outcomes Using Midazolam and Hydroxyzine with and Without Meperidine Book Summary : Background: Procedural sedation is an advanced pharmacological behavior guidance technique frequently used in the pediatric dental setting. It can be offered as an option for dental treatment for a young, potentially uncooperative pediatric patient to safely and effectively complete dental restorative needs. However, there are no standard regimens or drug dosages used among pediatric dentists, and sedation outcomes vary widely. Purpose: The primary objective of this randomized trial was to assess the effects of oral sedation using midazolam and hydroxyzine with and without meperidine on sedation outcomes in pediatric dental patients. The relationship between child temperament and sedation outcomes was also assessed. Methods: This pilot study recruited 14 children between the ages of 3-7 who met study eligibility criteria and were planned to undergo dental treatment with oral sedation at the University of Washington Center for Pediatric Dentistry. The children were randomly assigned to receive a regimen of midazolam and hydroxyzine with or without meperidine. Parents completed the Child Behavior Questionnaire Short Form (CBQ-SF) to assess temperament. Results: Out of 14 participants, there were no significant differences in sedation outcome in regard to age, sex, insurance status, sedation regimen, isolation method, treatment complexity, and procedure. There were also no significant differences in the effect of child temperament on overall sedation outcome, although within the non-narcotic regimen children with sedation failures displayed higher levels of both effortful control and extraversion/surgency. Conclusion: Further data collection is needed to assess the effect of procedure variables and child temperament on sedation outcomes. Conclusions regarding these relationships cannot be drawn at this time due to the small sample size. However, results from this study will contribute to identification of best practices when choosing a sedation regimen and improve our ability to identify optimal candidates for pediatric procedural sedation.

✏Why Do Palliative Care Patients Request Deep Sedation at Home in France and how Is Deep Sedation Conducted A Multicenter Observational Case control Study Book Summary : Deep sedation for palliative care patients was legalized in France in 2016. It is a difficult practice because the patients lose their capacity to communicate and are no longer autonomous. This can be stressful for doctors, nurses and caregivers who can feel deeply isolated. We tried to determine the reasons for sedation requests to further anticipate them and maybe avoid them. A secondary aim was to determine how deep sedations were conducted at home.We conducted a multicenter, observational case-control study in patients who chose to die at home, and who requested deep sedation. All home palliative-care teams and homecare services in an area of 2.5 million people were associated with this study in order to include patients and control patients, and to collect data according to our questionnaire. Data including socio-demographics, spiritual faiths, symptoms, prognosis, advance-care directives, medical evaluations, deep-sedation representation and deep-sedation parameters were collected.20 patients were included over a 3 month-period. 11 had deep sedation and 9 were control patients. The only significant difference between the 2 groups was anxiety. Anxiety was present among 81.8 % of cases vs 22.2 % of control patients (p 0.0216). There were no significant differences for dyspnea, pain, digestive symptoms, insomnia, depression. 5 deep sedations were started according to advance-care directives. Deep sedation was conducted with midazolam in 90.9 % of patients, clorazepate in 18.2 %, and both drugs in 9.1 %. Different midazolam doses were used at daytime and nightime in 2 patients. Titration was used in 5 out of 11 patients. Midazolam boluses were necessary in 3 patients.In this study, anxiety is the only significant factor for deep-sedation requests in home patients. Therefore home medical-teams would focus on anxiety in order to better accompany and treat palliative-care patients who choose to die at home. Home deep sedation is mainly conducted with midazolam.
