✏Restless Legs Syndrome Book Summary : Restless legs syndrome (RLS), also known as Ekbom syndrome, is a common movement disorder with sensorimotor symptoms occurring during sleep and quiet wakefulness. Yoakum described RLS as the "most common disorder you've never heard of" and this may, unfortunately, be an appropriate description of RLS. The term "restless leg syndrome" was first introduced by Karl-Axel Ekbom, a Swedish neurologist and surgeon in 1945. RLS can present itself in primary are and secondary care, across a range of specialties, such as psychiatry, rheumatology, and sleep medicine, and in the UK, the condition remains under-recognized and often regarded as a neurosis in spite of evidence that RLS adversely affects quality of life. Inappropriate clinical history taking leads to misdiagnosis and under diagnosis and a sense that the condition may be a "manufactured" one. Although RLS is effectively treatable and two drugs are now specifically licensed for RLS in the UK, the condition is generally poorly treated and investigated and often prescribed inappropriate drugs. Because of such a broad, unusual spectrum of reported sensations, RLS is frequently misunderstood and misdiagnosed, and even classified as a psychogenic disorder. RLS has been described during and after spinal anesthesia using bupivacau00efne en lidocaine, the mechanism behind these symptoms is not yet understood.MethodsIn this case series we present three patients who experienced restless legs during spinal anesthesia using hyperbaric prilocaine. None of our three patients received anti-dopaminergic drugs, or suffered from any of the medical conditions associated with an increased risk for RLS.ResultsIn these three patients we did not treat the restless legs, but provided sedation in order to comfort and to overcome the discomforting symptoms. Moreover, we reassured our patients that the symptoms would diminish when the spinal anesthesia would wear off, which was the case in all patients.ConclusionsThis is the first case series to describe RLS symptoms during spinal anesthesia with hyperbaric prilocau00efne. The mechanism is unclear, but restless legs have been described before after intrathecal injection of lidocaine and bupivacaine. The restless legs symptoms disappear when the motor block wears off.

The aim of this study was to determine the relationship between RLS and morning-eveningness in Korean adolescents. MethodsOf the 867 community-dwelling high school students, 590 subjects were included in this study. All participants completed self-report questionnaires, including demographic variables, particulars about menstruation, life style, sleep duration, RLS severity, Composite scale (CS), Epworth sleepiness scale (ESS), Insomnia severity scale (ISI), Patient health questionnaire-9 (PHQ-9), Pittsburgh sleep quality index (PSQI) and Berlin questionnaire (BQ). ResultsParticipants with RLS symptoms had a higher prevalence of depression, defined by a PHQ-9 score u226510 (OR 3.03, CI 1.11u20138.26) and eveningness in chronotype (OR 1.95 CI 1.15u20136.43) when adjusted for depression, excessive daytime sleepiness, insomnia symptom and morningness-eveningness. However, RLS symptoms had no association with excessive daytime sleepiness and insomnia, when adjusted for clinical factors.ConclusionsIn Korean high school students, restless leg symptom may be preceded by depression and eveningness of chronotype. For healthy sleep lifestyle in the adolescents, an appropriate evaluation of RLS symptom as well as chronotype is recommended.

RLS is also a common adverse effect of some antidepressant medications; however, it is underdiagnosed when associated with second-generation antipsychotics prescription. Objectives: This review aims to update the scientific advances on the pathophysiology of RLS and explore the association with quetiapine.Methods: We performed a literature search in Medline and PubMed with keywords u201crestless legs Syndrome quetiapineu201d in September 2018. We present a non-systematic review of the literature that approach RLS pathophysiology and the RLS symptoms induced by quetiapine.Results: Approximately 2.5% to 15% of the general population and increases with age and with a female preponderance. There is increasing evidence for the role of brain iron insufficiency, dopamine neurotransmission abnormalities and hypodopaminergic states in the nigrostriatal circuits, are in the development of RLS. RLS overlaps Psychiatry in three important areas: co-morbid disorders, dopamine-induced akathisia, and medications exacerbating RLS.We found a total of 14 case reports that describe RLS symptoms associated with quetiapine small doses (50-600 mg at bedtime) in a dose-dependent way, and even after a single low dose quetiapine administration. Although quetiapine is known for its low antidopaminergic activity, it was also associated with RLS cases previously.Conclusion: The exact mechanisms by which abnormalities in dopaminergic neurotransmission or in dopamine metabolism result in the development of RLS remain unclear. Quetiapine seems to carry a special risk for RLS although this association remains quite rare. More research is needed to clarify the underlying mechanisms of antipsychotic-induced RLS.

Backgrounds The association of primary headaches, especially of migraine, with RLS has recently attracted much attention. Migraine prevalence was reported to be higher in patients with RLS than in the general population, and the role of dopamine was strengthened. Methods We evaluated 265 consecutive adult RLS patients (137 males and 128 females) followed up in a Sleep Disorders Unit and diagnosed according to criteria defined by the International Restless Legs Syndrome Study Group (IRLSSG). RLS characteristics, and the severity, were performed by using the IRLSSG severity scale. The diagnosis of headache subtypes was defined by the International Classification of Headache Disorders. Gender, age, age at RLS onset, duration of RLS, family history of RLS, family history of headache, presence of depression, any treatments given for RLS, and the change in headache following RLS treatment were questioned. Results The mean age of the study population was 50.4 ± 12.8 years, mean age at RLS onset was 41.6 ± 13.2 years, and mean disease duration was 8.40 ± 8.6 years. Of these, 163 patients had headache; 40 of them were diagnosed to have migraine‐type headache (15.1%). The presence of migraine‐type headache was 9.4% in males with RLS, and 21.1% in female RLS patients. In RLS patients with migraine, 67.5% were females, while 48.0% of RLS patients with other types of headache were females (P = .032), and only 41.2% of RLS patients without headache were females (P = .005). This systematic review assesses the current complementary and alternative options for RLS and the potential benefits of those treatments on sleep quality, mood disorder, and quality of life. A systematic search of the PubMed, Embase, Cochrane, and Web of Science databases was conducted. Eighteen studies met the inclusion criterion, which included the use of the international RLS study group criteria. Complementary and alternative therapies have been found to be effective in both primary and secondary RLS. The severity of primary RLS symptoms can be significantly ameliorated by exercise training, transcutaneous spinal direct current stimulation, pneumatic compression devices, light therapy, repetitive transcranial magnetic stimulation, and acupuncture. Pneumatic compression devices and yoga also improve RLS-related disorders. Exercise training is highly efficacious in the reduction of symptom severity in uremic RLS and related effects such as poor quality of life. Endovenous laser ablation may be a good choice for patients with concurrent RLS and superficial venous insufficiency.

✏Identification of the Molecular Mechanisms Underlying Restless Legs Syndrome Book Summary : "Restless legs syndrome (RLS) is a sleep-related sensory-motor disorder. It is characterized by uncomfortable sensations in the legs, mostly during the evening or at night. These symptoms can partially be relieved by movement, so the patients have to stand up or walk in the middle of night at their rest time to alleviate the sensory symptoms. RLS consequences are decreased quality of life and the mental health. This disorder is highly frequent in the elderly in the European descent populations. Currently, RLS can be considered as one of the most common and probably least known neurological conditions. This is a complex disorder with both genetic and environmental factors contributing to its development. Genome-wide association studies (GWAS) have identified 19 loci associated with RLS, among which common variants within MEIS1 (Myeloid ecotropic viral insertion site 1) and SKOR1 (SKI family transcriptional corepressor 1) showed significant associations. Despite the high prevalence of this disease, and the several genetic studies performed on it, very little is known about the neurobiological mechanisms that lead to development of RLS. The work done in this thesis investigates the functions of two RLS-associated genes, i.e. MEIS1 and SKOR1. We showed that there are direct links between MEIS1 and SKOR1 where MEIS1 has a regulatory role over SKOR1 expression. Furthermore, we studied the transcriptome-wide regulatory role of MEIS1 in RLS using in vitro cell models and identified novel genes and pathways that are regulated by MEIS1, and possibly involved in RLS biology. Additionally, postmortem brain tissues of RLS individuals were implemented to validate the cell line findings. We observed that mineral absorption pathway was significantly enriched in both in vitro cell models and the brain tissues. Due to the crucial role of iron in RLS pathogenesis, enrichment of mineral absorption pathway is a meaningful finding in RLS research. Moreover, we also performed investigations on the regulatory role of SKOR1 in RLS and found pathways related to iron metabolism and also neurodevelopment. ✏Intertemporal Choice in Parkinson s Disease and Restless Legs Syndrome Book Summary : Abstract: Background: Impulse control disorders in Parkinson's disease are a potential consequence of dopaminergic therapy. Impulse control problems might be revealed by intertemporal choice tasks which entail to forgo an immediately available reward in favor of a larger but later reward. The steepness of the discounting curve can be quantified by the parameter k . Methods: Participants (37 Parkinson patients [13 de novo, 24 medicated], 24 patients with restless legs syndrome, and 22 controls) were offered 54 choices between immediate smaller rewards and delayed larger and the k value was estimated from the participants' responses. Participants had the chance of winning one of their decisions. None of the participants had impulse control disorders. Results: Unmedicated Parkinson patients had a higher discounting rate than controls and medicated patients with restless legs syndrome. The k values of medicated Parkinson patients and patients with restless legs syndrome did not differ from those of controls. No correlation was found between the k value and the dopamine agonist dose. Conclusion: Impulsive decision making in patients with Parkinson's disease may occur as part of the disease rather than as a consequence of dopamine agonist therapy. Highlights: De novo Parkinson patients show steeper delay discounting than healthy controls. Compared discounting functions across several groups of participants including restless legs and de novo patients. Steeper discounting is likely a trait marker of Parkinson's disease.
