✏COGNITIVE REHABILITATION AFTER GLOBAL APHASIA DUE TO TRAUMATIC BRAIN INJURY A CASE REPORT Book Summary : Global aphasia is a syndrome involving receptive and expressive language impairment. Treatment protocols varies, based on each individualu2019s unique language profile and communication needs. We referred to a 28-year-old patient, who was ddiagnosed with global aphasia due to traumatic brain injury. He was admitted to our department 3 months after his accident. An initial evaluation showed impaired auditory and reading comprehension , poor verbal expression , impaired repetition and difficulty with naming and with sentence completion. Additionally he showed spatial estimation, calculation memory (especially short) disorders.In this case, we tried a patient-oriented rehabilitation protocol, based on cognitive abilities. Short-term goals were the improvement of speech deficits, cognition, perception and visual spatial awareness. The long-term goals of this protocol centered on self-independence in activities of daily living, enhancement of memory and judgment, as well as improvement in writing and numerical concepts.A 2-hour treatment program was seated 5 times a week including traditional interventions (speech and occupational therapy and psychological support). Our program involved playing with cards, table games, drawing and a computer-based treatment (visual matching objects and pictures). Mini Mental State (MMS) was performed to evaluate patients cognitive ability every month. Independence was measured with Modified Barthel Index (MBI). The main target was to inhase memory and attention first followed by others cognitive skillsAt the beginning the first MMS evaluation was 4/10, the second 16/30, the third 18/30 and the last 24/30. First MBI was 10, the second 37, and the last 81.Early studies have shown that there is no specific rehabilitation protocol which results in a faster and near-total recovery in global aphasia. In this case we mainly focus on memory and attention followed by the rest rehabilitation programme, gaining better results in cognition and independence on ADLs.

✏Traumatic Brain Injury Rehabilitation Book Summary : In 1986 the Trustees of the Nuffield Provincial Hospitals Trust decided to develop a major core programme of studies concerned with the organization, development and improvement of services for physically disabled and frail elderly people. This programme developed rapidly due to the enthusiastic response of those in the field; and by 1992 the total value of the programme was in excess of £2 million and currently totals over £3. 2 million. It comprised a cohesive matrix of grants commissioned throughout the United Kingdom for work in identified areas which, taken in conjunction with existing grants, formed a comprehensive programme of much-needed studies and research in this field. The whole was to be underpinned by a major study planned and financed by the Trust but conducted by the Royal College of Physicians to survey the existing services being provided by some 180 health authorities. This study, together with concern for the handicapped schoolleaver, information about the availability of disability services and the care of mild/moderate head-injured persons, formed the prime initial thrust of the programme. In the case of head injury, the Trust was particularly concerned that there was discontinuity of care for many patients and little evidence as to which treatments were effective. Once patients had stabilized in hospital they were discharged into the community under the care of their general practitioner often without a continuing responsibility from the hospital.

✏Cognitive Rehabilitation After Traumatic Brain Injury Book Summary : Nearly 1.7 million Americans sustain a traumatic brain injury (TBI) each year . These injuries can result in physical, emotional, and cognitive consequences. While many individuals receive cognitive rehabilitation from occupational therapists (OTs), the interdisciplinary nature of TBI research makes it difficult to remain up-to-date on relevant findings. We conducted a literature review to identify and summarize interdisciplinary evidence-based practice targeting cognitive rehabilitation for civilian adults with TBI. Our review summarizes TBI background, and our cognitive remediation section focuses on the findings from 37 recent (since 2006) empirical articles directly related to cognitive rehabilitation for individuals (i.e., excluding special populations such as veterans or athletes). This manuscript is offered as a tool for OTs engaged in cognitive rehabilitation and as a means to highlight arenas where more empirical, interdisciplinary research is needed.
