✏Towards Offline PET Monitoring at a Cyclotron Based Proton Therapy Facility Book Summary : Matthias Würl presents two essential steps to implement offline PET monitoring of proton dose delivery at a clinical facility, namely the setting up of an accurate Monte Carlo model of the clinical beamline and the experimental validation of positron emitter production cross-sections. In the first part, the field size dependence of the dose output is described for scanned proton beams. Both the Monte Carlo and an analytical computational beam model were able to accurately predict target dose, while the latter tends to overestimate dose in normal tissue. ✏Proton Therapy for Craniopharyngioma An Early Report from a Single European Centre Book Summary : Abstract: Aims: Proton beam therapy (PBT) is being increasingly used for craniopharyngioma. We describe our early outcome of patients treated with PBT. Materials and methods: Between August 2013 and July 2016, 18 patients with craniopharyngiomas were treated with 54Cobalt Gray Equivalent (CGE) in 30 fractions over 6 weeks at our centre. The early outcome of 16 patients included in a registry study was analysed. Radiological response was assessed by RECIST criteria and the disease- and treatment-related toxicities were scored according to the CTCAE 4.0. Results: All patients are alive at a median follow-up of 32.6 months (range 9.2–70.6 months) from initial diagnosis. The median age at PBT was 10.2 years (range 5.4–46.9 years). One patient progressed 8.7 months after PBT and subsequently had complete resection of the tumour. At a median follow-up of 18.4 months after PBT, five patients remained in complete remission, four in partial remission and seven with stable disease. The most common adverse effects during PBT were grade 1 (cutaneous in seven patients and fatigue in six patients). There were no treatment-related grade 3 toxicities. Conclusions: Our early results are encouraging and comparable with the limited literature on PBT for craniopharyngioma. Highlights: Proton therapy (PBT) is being increasingly used for craniopharyngioma. Encouraging early results of PBT for craniopharyngioma correspond to published data. A systematic evaluation of neurocognitive function before PBT is recommended.

The dose of radiotherapy was 25 CGE/5 fractions. Pencil beam proton therapy system (Sumitomo Heavy Industries) using single-field uniform dose (SFUD) was delivered through 2 posterior oblique fields (135 and 215 degree). Doses of pelvic bone marrow, small bowel, and penile bulb were calculated using RayStation 6.0 planning system. Image-guided cone beam CT was used before each treatment. Acute toxicities of radiotherapy were evaluated by common toxicity criteria (CTC) version 4.Results: Dosimetries of pelvis bone marrow (V2.5=45.9u00b112.0% vs. 85.6u00b19.1%; V5=40.4u00b110.5% vs. 79.6u00b19.8%; V7.5=35.8u00b18.6% vs. 75.7u00b110.3%; V10=29.7u00b14.9% vs. 62.8u00b17.9%), small bowel (V10=58.6u00b139.3 mL vs. 142.9u00b194.0 mL; V15=50.3u00b134.3 mL vs. 73.9u00b137.4 mL; V20=41.5u00b128.4 mL vs. 47.3u00b130.6mL), and penile bulb (mean dose=6.4u00b11.7 CGE vs. 12.1u00b11.3 CGE) were better than VMAT planning. One patient completed proton therapy then received surgery within 1 week. No Grade 1 or greater leukopenia, dermatitis and diarrhea were noted before or after operation. Conclusion: Short-course preoperative pencil beam proton therapy for rectal cancer is feasible because of better dosimetries of pelvic bone marrow, small bowel, and penile bulb. Further prospective study is considered for short-term and long-term outcomes.

It could avoid direct shooting to brain stem and keep distal uncertainty far away from the most critical organ, which is brain stem. However, the radiation doses in the proximal part of proton beam are still high that makes a certain dose to bilateral temporal lobe when lateral opposite irradiation applying. Temporal lobe radiation injury may cause memory and cognitive function impairment that effect the quality of life. In this study, we evaluated the alternating wide-angle arrangement proton beams to skull base lesion in phantom to evaluated the effectiveness to sparing temporal lobe comparing to bilateral opposite direction. The alternating wide-angle proton beam angles were paired as 240-90 degrees and 120-270 degrees for treatment each day. One day treated with 240-90 degrees beams and 120-270 degrees on the other for the treatment course (Figure 1). With the prescribed dose of 69.96GyE/33 Fractions to skull base lesion after optimization, the results showed that the mean doses of right temporal lobe were 41.78GyE and 39.40GyE for alternating wide-angle and bilateral opposite beam arrangements. The mean doses of left temporal lobe were 43.08GyE and 40.29GyE for alternating wide-angle and bilateral opposite beam arrangements. Although the iso-dose coverage area of temporal lobe seems to less by alternating wide-angle proton beam arrangement in visual intuition, the alternating wide-angle proton plan did not demonstrate a numerical benefit for temporal lobe in dose volume histogram evaluation in this study.

✏10 Scanning Beam Proton Therapy Versus Photon IMRT For Stage III Lung Cancer Comparison of Dosimetry Toxicity and Outcomes Book Summary : Purpose: There is limited clinical data on scanning-beam proton therapy (SPT) in treating locally-advanced lung cancer, as most published literature is with passive-scatter technology. There is increasing interest in whether the dosimetric advantages of SPT compared with photon therapy can translate into superior clinical outcomes. We present our experience of SPT and photon intensity-modulated radiation therapy (IMRT) with real-life dosimetry, and outcomes in patients with stage III non-small cell lung cancer (NSCLC). Methods: Patients with stage III NSCLC treated at our center between 2013-May 2018 were identified in compliance with an IRB-approved study (64 patients=34 SPT +30 IMRT). Most proton patients were treated with pencil-beam-scanning (28/34), 6/34 with uniform-scanning. Fisheru2019s exact test, Chi-square test, and Mann-Whitney test were used to compare groups. All tests were two-sided.Results: Patient characteristics are in Table 1. Mean dose to lung, heart, and esophagus were lower in the SPT group, with most benefit in the low dose region (Table 2). Esophagitis and dermatitis grades were not different between the two groups (Table 3). Grade 2+ pneumonitis rate was 21% in the SPT group and 40% in the IMRT group (p=0.1). Overall survival and progression free-survival were not different between SPT and IMRT. Conclusions: We report our experience with scanning beam proton therapy and photon IMRT in stage III NSCLC, showing lower dose to normal organs (lungs, heart, esophagus) with SPT than IMRT. There is no statistically significant difference in toxicity rates or survival, although there may be a trend towards lower rates of pneumonitis.

✏Dual energy Cone beam CT for Proton Therapy Book Summary : Proton therapy is a promising radiation treatment modality that uses proton beams to treat cancer. Current treatment planning systems rely on an X-ray computed tomography (CT) image of the patient's anatomy to design the treatment plan. The proton stopping-power ratio relative to water (SPR) is derived from CT numbers (HU) to compute the absorbed dose in the patient. Protons are more vulnerable than photons to changes in tissue SPR in the beam direction caused by movement, misalignment or anatomical changes. In addition, inaccuracies arising from the planning CT and intrinsic to the HU-SPR conversion greatly contribute to the proton range uncertainty. In clinical practice, safety margins are added to the treatment volume to account for these uncertainties at the expense of losing organ-sparing capabilities. The use of dual-energy (DE) in proton therapy was first suggested in 2009 to better estimate the SPR with respect to single-energy X-ray imaging. The aim of this thesis work is to investigate the potential improvement in determining proton SPR using DE to reduce the uncertainty in predicting the proton range in the patient. This PhD work is applied to a new imaging device, the Imaging Ring (IR), which is a cone-beam CT (CBCT) scanner developed for image-guided radiotherapy (IGRT). The IR is equipped with a fast kV switching X-ray source, synchronized with a filter wheel, allowing for multi-energy CBCT imaging. The first contribution of this thesis is a method to calibrate a model for the X-ray source and the detector response to be used in X-ray image simulations. It has been validated experimentally on three CBCT scanners. Secondly, the investigations have evaluated the factors that have an impact on the outcome of the DE decomposition process, from the acquisition parameters to the post-processing. Both image- and projection-based decomposition domains have been thoroughly investigated, with special emphasis on projection-based approaches. Two novel DE decomposition bases have been proposed to estimate proton SPRs, without the need for an intermediate variable such as the effective atomic number. The last part of the thesis proposes an estimation of proton SPR maps of tissue characterization and anthropomorphic phantoms through DE-CBCT acquisitions with the IR. A correction for X-ray scattering has been implemented off-line, and a routine to linearly interpolate low-energy and high-energy sinograms from sequential and fast-switching DE acquisitions has been proposed to perform DE material decomposition in the projection domain with real data. DECT-derived SPR values have been compared with experimentally-determined SPR values in a carbon-ion beam.
