✏Biomarkers in Renal Disease Book Summary : Advances in long-term improvement and outcomes of patients with kidney disease will require the use of novel biomarkers to identify patients at high risk for kidney disease and to diagnose kidney disease early for effective treatment. A biomarker is a substance found in the blood, body fluids or tissues that provides a measure of normal biological or pathological processes or response to pharmacological compounds or drugs. There are a wide variety of biomarkers including but not limited to mRNA, proteins and peptides, and lipid molecules. In AKI, important pathophysiological processes such as inflammation, apoptotic and necrotic cell death and, tubule regeneration may be reflected in blood or urine. An array of candidate markers along with clinical information in long-term clinical studies with appropriate analytical methodologies will likely provide prognostic information. Despite well-known limitations, currently the most widely used biomarkers for the early diagnosis of CKD and AKI are proteinuria, serum creatinine and blood urea nitrogen. Most clinicians are aware that serum creatinine and blood urea nitrogen are poor biomarkers due to inherent characteristics of these molecules and handling by the kidney. Creatinine is secreted and urea nitrogen is reabsorbed by the renal tubules. Many endogenous substances interfere in the assay for creatinine. Serum creatinine and urea appear late after acute kidney injury and the serum levels in part depend on the generation (large or small body mass). Acute kidney injury is a non steady state condition thus serum creatinine and urea nitrogen will lag behind kidney injury. For these reasons new biomarkers are imperative. With knowledge of these limitations in use of current biomarkers and the lack of progress in reducing the mortality and morbidity from kidney disease, there has been a great surge of interest in identifying novel biomarkers with a particular emphasis on the early diagnosis of kidney disease. A variety of methods have been employed including transcriptomics, proteomics, gene arrays and lipidomics. Currently, candidate biomarkers have been found in different disorders and have been tested in humans and many candidate biomarkers have yet to be identified. Most studies to date are preliminary and require validation in large multicentre studies followed by commercial assay development validation and testing. This new book outlines the rapid advances made in the field of biomarker development for kidney disease in which a variety of novel molecules have been identified and studied in humans.

✏Urine Proteomics in Kidney Disease Biomarker Discovery Book Summary : This book systematically summarizes the ideas and technologies used in urine proteome analysis. It argues that change is the core of biomarker definition since the body uses its homeostatic mechanisms to correct changes in the blood. This means that urine is probably a better source of biomarkers than blood. A roadmap to the urinary biomarker era is proposed, and researchers are reminded of the potential opportunities and risks in their study design. Kidney diseases are emphasized as they produce the most significant changes in urine. This book tries to show researchers and graduate students, who are in or entering the field, "all things considered" rather than "the current affair".

✏Urinary Biomarkers Improve the Diagnosis of Intrinsic Acute Kidney Injury in Coronary Care Units Book Summary : Abstract : Supplemental Digital Content is available in the text Abstract : Abstract: Acute kidney injury (AKI) is associated with increased morbidity and mortality and is frequently encountered in coronary care units (CCUs). Its clinical presentation differs considerably from that of prerenal or intrinsic AKI. We used the biomarkers calprotectin and neutrophil gelatinase-associated lipocalin (NGAL) and compared their utility in predicting and differentiating intrinsic AKI. This was a prospective observational study conducted in a CCU of a tertiary care university hospital. Patients who exhibited any comorbidity and a kidney stressor were enrolled. Urinary samples of the enrolled patients collected between September 2012 and August 2013 were tested for calprotectin and NGAL. The definition of AKI was based on Kidney Disease Improving Global Outcomes classification. All prospective demographic, clinical, and laboratory data were evaluated as predictors of AKI. A total of 147 adult patients with a mean age of 67 years were investigated. AKI was diagnosed in 71 (50.3%) patients, whereas intrinsic AKI was diagnosed in 43 (60.5%) of them. Multivariate logistic regression analysis revealed urinary calprotectin and serum albumin as independent risk factors for intrinsic AKI. For predicting intrinsic AKI, both urinary NGAL and calprotectin displayed excellent areas under the receiver operating characteristic curve (AUROC) (0.918 and 0.946, respectively). A combination of these markers revealed an AUROC of 0.946. Our result revealed that calprotectin and NGAL had considerable discriminative powers for predicting intrinsic AKI in CCU patients. Accordingly, careful inspection for medication, choice of therapy, and early intervention in patients exhibiting increased biomarker levels might improve the outcomes of kidney injury.

✏Acute Kidney Injury Book Summary : Acute kidney injury is defined as an abrupt change in serum creatinine and/or urine output, and a majority of patients admitted to the ICU have some evidence of the disorder. Unfortunately, treatment for this complex syndrome is as yet lacking and understanding is limited. An interdisciplinary panel of experts has contributed to this volume, illuminating some of the fundamental and complex aspects of the disorder ranging from pathophysiology to treatment, from emerging biomarkers to genetic polymorphisms. Other contributions focus on immunological issues or the many complications of acute kidney injury and co-morbid conditions encountered, covering the fundamentals as well as the latest developments. Moreover, important technical aspects of extracorporeal therapies including vascular access, anticoagulation or fluid composition are introduced, and different approaches to renal support from intermittent dialysis to continuous therapies and hybrid techniques are discussed. A description of advanced extracorporeal techniques of organ support and their role in the management of sepsis and acute kidney injury in the context of an overall strategy of multi-organ failure management concludes the discussions. This volume not only provides a practical and up-to-date summary of current knowledge and technology, but also imparts a fundamental understanding of the pathogenesis and likely future developments in this field. It also serves to challenge and re-examine the fundamental underlying assumptions we hold regarding critical illness in general and acute kidney injury in particular.

✏ADQI Consensus on AKI Biomarkers and Cardiorenal Syndromes Book Summary : Associated with both acute kidney injury (AKI) and cardio-renal syndromes (CRS), new biomarkers represent both a popular area of investigation and a new opportunity for advancement of therapy. This book contains the resolutions of the most recent ADQI conferences on biomarkers in AKI (Dublin) and on cardio-renal syndromes (Venice). The first part answers specific questions about new biomarkers and their use and utility in AKI: What are the most suitable candidate molecules and physiologic measures, how solid and evidence based is the discovery phase? How can we incorporate the new biomarkers in the AKI conceptual model describing the evolution from susceptibility to insult, decreased GFR and organ death? Even if we have a positive biomarker pattern and we can identify patients at risk or patients with early or even subclinical AKI, how is this information affecting our clinical behavior and practice? The second part is dedicated to the appraisal of the current knowledge about the pathophysiological mechanisms involved in different forms of CRS: it contains contributions on the state-of-the-art knowledge and practice of CRS, particularly focusing on the pathophysiology of the five subtypes. Acute and chronic mechanisms of damage are explored in depth, with particular attention to the primacy of organ involvement and the subsequent pathways of organ crosstalk.Presenting the most recent research in the field of biomarkers, AKI and CRS, this publication is an important educational tool for advanced investigators and clinical experts, but also for students and fellows.

✏TIMP 2 and IGFBP7 as Early Biomarkers of Acute Kidney Injury After Orthotopic Liver Transplantation Book Summary : Background and goal of study: Acute kidney injury (AKI) is a frequent complication after orthotopic liver transplantation (OLT) and it`s early detection is of crucial importance for an appropriate management. Since the traditional kidney function parameters like serum-creatinine (s-Cr) have severe limitations concerning early detection of AKI, novel biomarkers such as TIMP-2 and IGFBP7, have been studied and shown to be superior in previous studies in critically ill patients. The aim of our study was to evaluate the predictive value of these novel biomarkers during the early perioperative period after human OLT. Materials and Methods: Paired urine and blood samples from patients undergoing OLT were collected at five different time points (TP): after induction of anesthesia (TP0), end of surgery (TP1) and at day 1 and 2 after OLT (TP2, TP3). The incidence of perioperative AKI was assessed according to the Acute Kidney Injury Network (AKIN) criteria. The detection of urinary TIMP-2 and IGFBP7 was performed using the NephroCheck (NC) test (Astute140u00aeMeter), an ELISA based assay, which defines the risk score for AKI. Data are presented as mean u00b1 standard deviation (SD). Results and Discussion: Forty liver transplant recipients were enrolled in this study. Sixteen (40%) patients developed AKI stage 2 or 3 within 48 h post OLT and 6 (15%) patients were supported with renal replacement therapy. There was no significant difference in the receiver operating characteristics curve analysis at TP0 and TP1 between the NC test and s-Cr in regard of development of AKI 2 or 3 within the first 48 hours post OLT. S-Cr at TP2 performed even better (AUC 0.83; CI 0.68-0.93; p=0.0001) than the NC test (AUC 0.51; CI 0.34-0.67; p=0.26). Only on day 2, both, s-Cr (AUC 0.90; CI 0.76-0.97; p=0.0001) and NC (AUC 0.70; CI 0.54-0.84; p=0.02) predicted AKI 2 and 3. Conclusions: TIMP-2 and IGFBP7 are not superior to the traditional kidney function parameters in predicting development of perioperative AKI II and III in clinical setting of human OLT.

✏Biomarkers in Kidney Disease Book Summary : In the past decade there has been a major sea change in the way disease is diagnosed and investigated due to the advent of high throughput technologies, such as microarrays, lab on a chip, proteomics, genomics, lipomics, metabolomics etc. These advances have enabled the discovery of new and novel markers of disease relating to autoimmune disorders, cancers, endocrine diseases, genetic disorders, sensory damage, intestinal diseases etc. In many instances these developments have gone hand in hand with the discovery of biomarkers elucidated via traditional or conventional methods, such as histopathology or clinical biochemistry. Together with microprocessor-based data analysis, advanced statistics and bioinformatics these markers have been used to identify individuals with active disease or pathology as well as those who are refractory or have distinguishing pathologies. New analytical methods that have been used to identify markers of disease and is suggested that there may be as many as 40 different platforms. Unfortunately techniques and methods have not been readily transferable to other disease states and sometimes diagnosis still relies on single analytes rather than a cohort of markers. There is thus a demand for a comprehensive and focused evidenced-based text and scientific literature that addresses these issues. Hence the formulation of Biomarkers in Disease. The series covers a wide number of areas including for example, nutrition, cancer, endocrinology, cardiology, addictions, immunology, birth defects, genetics, and so on. The chapters are written by national or international experts and specialists.

✏Biomarkers of Kidney Function and Cognitive Performance in Middle aged and Older Adults Book Summary : Background: As a consequence of an aging world population, cognitive impairment and dementia are growing public health concerns. Previous studies suggest that kidney dysfunction is associated with cognitive decline. Most studies were limited to one marker of kidney function, and the extent to which associations are modified by genetics has received little consideration. Moreover, whether these associations are causal is not clear. Methods: This dissertation is composed of three studies. Study one was a longitudinal study conducted among 1,634 participants from the Rancho Bernardo Study of Healthy Aging (RBS), who had repeated measures of kidney function and cognitive follow-up of up to 24 years. Study two was a cross-sectional study using data from up to 341,208 participants from the UK Biobank (UKBB) with three different measures of kidney function, genetic data, and cognitive function data from the baseline assessment. Study three used data from 357,590 UKBB participants to assess causal associations between biomarkers of kidney function and cognitive performance using a Mendelian Randomization (MR) approach. Results: In study one, men with albuminuria (urine albumin-to-creatinine ratio [ACR] ≥25 mg/g for men and ACR ≥30 mg/g for women) had faster cognitive decline across multiple domains. High serum uric acid (SUA) was related to lower baseline global cognitive function scores in men. There was no association between kidney function and cognitive performance in women. In study two, albuminuria, creatinine-based estimated glomerular filtration rate (GFRcre)
