✏Improving the Accuracy of Image guided Neurosurgical Tools for Brain Tumour Resections Book Summary : "Each year, thousands of Canadians undergo neurosurgery to areas of the brain that are critical to movement, vision, sensation, language and other cognitive functions. Recent literature demonstrates a significantly increased survival benefit with complete resection of primary and secondary brain tumours. Image guided neurosurgery correlates preoperative diagnostic patient images with the patient on the operating room table by using a patient-to-image registration and localizing and tracking the patient and a set of surgical instruments. There are many different types of errors that can arise during image-guided interventions related to technical, physical and biological factors that largely influence the accuracy of these systems and make them unreliable as the intervention progresses or when caution is not taken to minimize their effect.In the following dissertation, I explore the variety of different sources of errors that affect the accuracy of image-guided neurosurgical systems and establish ways in which some of them can be minimized to maximize the accuracy of these systems throughout a procedure. We begin by performing an exhaustive review of the work done investigating different sources of these errors and develop a taxonomy to better classify the sources of these errors and the state of the art techniques that aim to minimize these errors. Next, the contribution to localization, registration, and calibration errors from optical tracking systems and landmark-based registration procedures used in image-guided neurosurgery is quantified and we give suggestions to minimize their error contribution. Positioning of tools, tracking cameras and the selection of landmarks for patient-to-image registration are all important factors involved in minimizing the baseline inaccuracy contributions from image-guided neurosurgery hardware and can have major implications on system accuracy if care is not taken when using or performing them. Additionally, we explore the use of intraoperative ultrasound and augmented reality to compensate for physiological inaccuracies introduced through the phenomenon of brain shift and to maintain a high level of accuracy throughout an image-guided procedure. Finally, we show that intraoperative ultrasound is a useful tool for compensating for some of the misalignment issues when using augmented reality visualization and augmented reality complements the ultrasound by having more easily interpretable images for intraoperative visualization.The results of the error minimizing techniques described in this dissertation suggest that with continued development, and by exercising caution while setting up and using the technology image-guided surgery can realize its potential in providing a highly accurate navigation tool to assist with complicated surgeries. While complete resection has been shown to correlate with survival, the patient's quality of life is often weighed into the decision of resection extent leading to a "maximal safe resection" that sometimes results in incomplete tumour removal. With continued development and improved accuracy, these systems have the potential to be relied upon for longer durations of an operation and may push the boundaries of maximal safe resections." --

✏Visual Perception and Cognition in Image guided Intervention Book Summary : Surgical image visualization and interaction systems can dramatically affect the efficacy and efficiency of surgical training, planning, and interventions. This is even more profound in the case of minimally-invasive surgery where restricted access to the operative field in conjunction with limited field of view necessitate a visualization medium to provide patient-specific information at any given moment. Unfortunately, little research has been devoted to studying human factors associated with medical image displays and the need for a robust, intuitive visualization and interaction interfaces has remained largely unfulfilled to this day. Failure to engineer efficient medical solutions and design intuitive visualization interfaces is argued to be one of the major barriers to the meaningful transfer of innovative technology to the operating room. This thesis was, therefore, motivated by the need to study various cognitive and perceptual aspects of human factors in surgical image visualization systems, to increase the efficiency and effectiveness of medical interfaces, and ultimately to improve patient outcomes. To this end, we chose four different minimally-invasive interventions in the realm of surgical training, planning, training for planning, and navigation: The first chapter involves the use of stereoendoscopes to reduce morbidity in endoscopic third ventriculostomy. The results of this study suggest that, compared with conventional endoscopes, the detection of the basilar artery on the surface of the third ventricle can be facilitated with the use of stereoendoscopes, increasing the safety of targeting in third ventriculostomy procedures. In the second chapter, a contour enhancement technique is described to improve preoperative planning of arteriovenous malformation interventions. The proposed method, particularly when combined with stereopsis, is shown to increase the speed and accuracy of understanding the spatial relationship between vascular structures. In the third chapter, an augmented-reality system is proposed to facilitate the training of planning brain tumour resection. The results of our user study indicate that the proposed system improves subjects' performance, particularly novices', in formulating the optimal point of entry and surgical path independent of the sensorimotor tasks performed. In the last chapter, the role of fully-immersive simulation environments on the surgeons' non-technical skills to perform vertebroplasty procedure is investigated. Our results suggest that while training surgeons may increase their technical skills, the introduction of crisis scenarios significantly disturbs the performance, emphasizing the need of realistic simulation environments as part of training curriculum.
