In vitro oocyte maturation is an experimental technique, with poorer results than conventional IVF. For this reason, improving in vivo maturation could meliorate the reproductive outcome of these patients. We performed a retrospective, not interventional, study analyzing the difference in the number and percentage of mature oocytes retrieved in patients with more than 50% immature oocytes in a previous IVF cycle triggered with human chorionic gonadotropin (hCG) compared to the number and rate of mature oocytes retrieved in subsequent cycles, triggered with both gonadotropin-releasing hormone agonist (GnRH-a) and hCG. The number of mature oocytes retrieved with a dual trigger was significantly higher than that for hCG alone: 5.3 ± 3.6 (4.4-6.1) versus 2.4 ± 2.2 (2-2.9). The proportion of mature oocytes showed the same tendency (79.6% vs 43.6%). The implantation, clinical, and ongoing pregnancy rates were 17.3%, 26.9%, and 15.3%, respectively, for the hCG trigger and 30.8%, 43.6%, and 31.6%, respectively, for the dual trigger. In patients with a low percentage of retrieved mature oocytes, who were triggered with a combination of GnRH-a and hCG, the number and percentage of retrieved mature oocytes improved. The dual trigger also seemed to meliorate gestational outcomes after IVF.

At our institution, we observed cases of elevated beta-hCG in patients with GCT leading to diagnostic difficulty and in one case, concern for metastatic choriocarcinoma. This study aims to determine the frequency of beta-hCG expression in GCT and any relationship to clinical aggressiveness. We evaluated tissue expression of beta-hCG by immunohistochemistry with 58% of cases staining for beta-hCG. Additionally, 2 of 11 patients with available serum and/or urine beta-hCG measurements demonstrated elevated beta-hCG due to tumor. It is important to be aware of beta-hCG expression by GCT and the potential for elevated urine and serum beta-hCG levels in patients with GCT so as to avoid misdiagnosis of pregnancy or gestational trophoblastic disease.

✏Glycoprotein Hormones Book Summary : Advances in the field of glycoprotein hormones necessitated a second international symposium on this topic, held March 11 to 14, 1993, in Santa Barbara, California, and again sponsored by Serono Symposia, USA. The meeting was twofold in its concept: (1) the dissemination of the current research in the field and (2) honoring three scientists who have greatly contributed to this field, Drs. Harold Papkoff, Robert Ryan, and Darrell Ward, upon their retirements. We were honored to have Dr. John Pierre present as a participant at the meeting and also serving as master of ceremonies at the banquet. Certainly one of the highlights at the first meeting, held in 1989, was the cloning of the CG/LH receptor. This second meeting was also filled with insightful and innovative scientific presentations. Significant advances in the regulation of gene transcription of the gonadotropins were presented by Pamela Mellon, James Hoeffler, John Nilson, Bruce Weintraub, and Joel Habener. These presentations reflected major advances from the in the understanding concepts offered at the last meeting. Other advances of hormone structure/function included the continuing work by Joyce Lustbader on the determination of the three-dimensional structure of hCG, including a new approach using nonisotopic labels for 3-D and 4-D NMR imaging. New insights into biosynthesis of hCG were proposed by Irving Boime. Studies directed at identifying receptor contact regions were presented by Henry Keutmann and David Puett.
