✏Novel clinical applications of extracellular vesicles Book Summary : During the last years, the research on extracellular vesicles (EVs) has raised giving new insights into pathophysiology of several diseases. EVs are membrane-bound particles secreted by almost all cell types. Depending on their biogenesis and size they include exosomes, microparticles / microvesicles and apoptotic bodies. Characteristically, EVs carry markers from the source cell membrane and contain genetic material, lipids and proteins inside. They are known to play a role in cell-to-cell communication and to produce genotypic and phenotypic modifications in the target cell including: antigen presentation, apoptosis induction, cellular activation, inhibition or differentiation. In particular, increasing concentrations of EVs have been found in many diseases such as cancer, autoimmune and cardiovascular diseases, among others. Most of the studies in EVs are focused on the characterization of EVs compounds, identifying mechanism of action, their potential use as biomarkers, and few of them investigate a therapeutic usage. However, there are some issues to be achieved on the path to their clinical application. This research topic offers a common place to discuss current and novel clinical applications of EVs pointing on future directions. Isolation of EVs is performed by several strategies, having advantages and disatvantages over each other. As such, the method of EV isolation and in particular exosome isolation determines the quality and purity of obtained vesicles. In this chapter, extracellular vesicle isolation methods are evaluated with regard to their further use. Methods such as ultracentrifugation with different modifications, size exclusion chromatography, ultrafiltration, affinity and precipitation are compared with respect to the yield efficacy and purity of isolates. Furthermore, the advantages and disadvantages of different methods according to the purpose of use are revealed. Recent progress and remaining challenges in the isolation of EVs with regard to diagnosis and treatment is reviewed and discussed. In order to select the most suitable method researchers should clearly define purity, yield, quantity and quality requirements for exosomes, and consider disadvantages of distinct isolation methods.

These small vesicles are known to morphologically and functionally resemble virus particles from human immunodeficiency virus type I (HIV-I) and human T-lymphotropic virus type I (HTLV-I). My research has focused mostly on the development of a diagnostic technique to monitor the progression of prostate cancer and early diagnosis of cancer. Biofunctionalized silica nanospring mats were prepared to selectivity capture tumor-derived exosomes through the interaction of the ligand with prostate-specific membrane antigen on the exosomes. The result obtained from biocinchonic acid assay shows high capture efficiency for the ligand-activated silica nanostructures. The silica nanostructures provide a high surface area support for immobilization of biomolecules and the open structure of the mats improves the accessibility of the flowing solvent. Tumor-derived exosomes are secreted by cancerous known as a new resource for the biomarker to facilitate early diagnosis via a non-invasive procedure. This proposed work is expected to accelerate the discovery of diagnostic or prognostic biomarkers. Another application of nanotechnology has been investigated in chapter 4 and 5. The potential of a nanoparticle delivery system has been evaluated to improve methods of delivery of candidate peptide-based vaccines for paratuberculosis. Poly (lactic-co-glycolic acid) nanoparticles vector containing the peptide elicit the development of cytotoxic T cells with the ability to kill intracellular mycobacterium avium ssp. Paratuberculosis which is the causative agent of Johne's disease. The pathogen has a broad host range that includes ruminants and non-ruminant species, including humans. Because Johne's disease is so difficult to diagnose before it progresses to clinical disease, vaccination is the only viable approach for safeguarding cattle and humans from the dangerous effects of paratuberculosis.
