✏Diabetic neuropathy Book Summary : The diagnosis of diabetic neuropathy is based on the diagnosis of diabetes, typical symptoms and clinical findings, as well as exclusion of other causes leading to neuropathy. Neuropathy that affects the large fibres can be verified objectively by ENMG and small fibre neuropathy is revealed by quantitative sensory threshold testing. The loss of small nerve fibres may also be demonstrated by skin biopsy. Neuropathy may be the first sign of type 2 diabetes. The loss of protective sensation as a consequence of peripheral neuropathy is the most important risk factor of diabetic foot ulcer. Small fibre neuropathy may already emerge during the phase of impaired glucose tolerance. 11–25% of the patients may have peripheral neuropathy in the pre-diabetic phase already.

✏Diabetic Neuropathy Book Summary : Comprised of contributions from a wealth of experts worldwide, this volume summarizes the latest research findings and new developments in diagnosis and treatment of diabetic neuropathy. While the actual cause of diabetic neuropathy, a common complication of diabetes mellitus, is not known, it is likely that no single mechanism predominates--rather that various mechanisms interact and culminate in the expression of neuropathy. The progressive nature of diabetic neuropathy and the high morbidity associated with it emphasize the need for early identification of patients at risk, particularly since the disease, if diagnosed early, can be corrected or controlled by intensified insulin treatment and strict metabolic control.

✏Pathophysiology of Pain Perception Book Summary : Under certain pathological conditions, the pain processing system, which includes both physiological and psychological components, can produce the experience of pain in the absence of any peripheral noxious event. Considerable evidence now indicates that alterations in pain perception are characteristic of many clinical pain states. Pathophysiology of Pain Perception primarily examines these pathological alterations in the pain-signaling system, with contributions from leading researchers in the field focusing on the functioning of the pain processing system under normal and pathological conditions. The understanding of pain perception is essential for optimal diagnosis and treatment of acute and chronic pain. This book covers the basics of pain perception (peripheral and central neurotransmission, methods of assessment), various pathological (chronic pain conditions, neurological and psychiatric diseases, cardiovascular disease), and non-pathological alterations of pain perception (pregnancy, labor) as well as the treatments aiming at influencing pain perception such as pharmacological, physical and psychological treatments.

✏Diabetic Neuropathy and Encephalopathy Book Summary : Diabetes mellitus produces numerous neurophysiological & structural changes in the central and peripheral nervous system and it further leads to development of neuropathic pain & moderate cognitive deficits in diabetics. The etiology of diabetes associated neuropathic pain & cognitive decline is multifactorial and involves an underlying phenomenon of metabolic memory. The concept of "metabolic memory," that is of diabetic vascular stresses persisting even after stringent glucose control, has been supported both in the laboratory and in the clinic in both type 1 & type 2 diabetes. Therefore, "switching off" the metabolic memory, could be an important strategy for the prevention of diabetic neurological complications. Insulin alone reverses the hyperglycemia but partially reversed neuropathic pain and memory deficits in diabetic rats. However, insulin in combination with tocotrienol, lycopene and sesamol not only attenuates the diabetic condition but also reverses neuropathic pain and memory loss by switching off various components of metabolic memory phenomenon. Thus, these interventions may find clinical application in therapeutic armamentarium of diabetics.

✏Diabetic Foot ECAB Book Summary : Of all lower extremity amputations, 40–70% are related to diabetes. In most studies, the incidence of lower leg amputation is estimated to be 5–25/100,000 inhabitants/year: among people with diabetes the number is 6–8/1,000. Lower extremity amputations are usually preceded by a foot ulcer in people with diabetes. The most important factors related to the development of these ulcers are peripheral neuropathy, foot deformities, minor foot trauma, and peripheral vascular disease. The spectrum of foot lesions varies in different regions of the world due to differences in socioeconomic conditions, standards of foot care and quality of footwear. This clinical update is designed to address this condition in a comprehensive way to help the reader take important questions while managing the patient with supportive typical clinical scenarios, with which all readers will be able to identify. Thus it provides an excellent opportunity to widen one’s perspective in this area.
