The impact of genetic determinants on the pathophysiology of type 2 diabetes is covered, as are chapters that address specific genes that are important for the development of type 2 diabetes. Finally, syndromic forms of diabetes, including Maturity Onset Diabetes of the Young and mitochondrial diabetes, and the insight that these disorders provide into more common forms of diabetes are reviewed. ✏Diabetes Mellitus Book Summary : Diabetes mellitus is an ancient disease that continues to plague modern man. Although the frequency of many serious disorders, such as coronary heart disease, has declined since the 1960's, diabetes rates are approaching epidemic proportions in Westernized countries. Although representing only 10% of cases of diabetes, type 1 diabetes is also rising in frequency. The economic cost of diabetes is staggering, costing the United States more than $100 billion annually. Appropriate management of diabetes requires the efforts of clinicians, laboratorians, and pharmacologists. The goal of this book is to acquaint the laboratorian with the pathophysiology, the diverse etiologies, the complications, the clinical management, and, finally, the diagnostic and routine laboratory evaluation of diabetes.

✏Molecular Pathology of Type 1 Diabetes Mellitus Book Summary : Type 1 diabetes as well as multiple sclerosis are thought to be T cell mediated autoimmune diseases that involve a detrimental action of inflammatory cytokines and autoaggressive T lymphocytes. They still pose many unsolved puzzles, and the precise etiology as well as prevention have remained elusive. It is clear that genetic factors can predispose for developing diabetes, however, based on significant disease discordance found in monozygotic twins, additional environmental factors have to be postulated. Viruses are good candidates because they induce strong cellular and humoral immune responses, but no single etiologic agent has been identified. Several animal models are presented which have been used to study the activation of naive autoreactive lymphocytes. It is shown that regulation of the autoaggressive process occurs prior to clinical diabetes and is mediated by a complete network of cytokines, as well as regulatory circuits/cells. The spreading of autoimmunity to self-antigens not involved in the initial phase of islet destruction is not necessarily detrimental and can carry benefits. Therapeutically, counter-regulation of aggressive responses has been demonstrated via various means in animal models preventing diabetes or rejection of transplanted islets.

✏Diabetes Mellitus Book Summary : People with diabetes mellitus experience health-literacy and self-management problems from the first days of diagnosis. As the tedium of detail and the continuous responsibility for well-being are realized, the self-efficacy of the individual is challenged (Polonsky, 1999). In addition, an exhaustion of physical, mental, and emotional strength may eventually impair motivation to self-manage this chronic illness. Stress often results from the continuing challenges required for successful management of the disease (Sperry, 2008). Management decisions are stressful as the diabetic strives to balance medication, diet, and exercise in order to reduce co-morbidities, complications, and end-stage progression. Because of the stresses involved, many diabetics suffer psychologically for years before experiencing the physical complications of the disease. More people die yearly from diabetes than from breast cancer and AIDS together (ADA, 2010). Diabetes management products, and their illusion of hope for living with diabetes, are not matched with the challenge of integrating health-literacy and self-management skills (Polonsky, 1999).
