Recent findings have provided some preliminary data showing that brain injury and drug abuse insults may share common pathway(s)leading to neuronal cell injury. Such pathways may be mediated by the activation of the protease families: the pronecrotic calpain and the proapoptotic caspase systems. These proteases can act on a number of substrates including the cytoskeletal proteins resulting in the breakdown products of such proteins can be used as biomarkers for brain injury insult (mechanical and chemical). The field of proteomics has been introduced as a new discipline for biomarker identification that allows for deciphering protein expression at very low concentrations. Having this in mind, neuroproteomics approach has been applied to identify the differential alterations of protein markers involved in two models of experimental traumatic brain injury as well as in drug abuse which showed high similarity in the underlying mechanisms leading to neuronal injury.

Although guidelines for the management of patients have been developed and more than 200 clinical trials have been conducted, they have resulted in few improvements in clinical outcomes and no effective therapies approved for TBI. It is now apparent that the heterogeneity of clinical TBI is underlain by molecular phenotypes more complex and interactive than initially conceived and current approaches to the characterization, management and outcome prediction of TBI are antiquated, unidimensional and inadequate to capture the interindividual pathophysiological heterogeneity. Recent advances in proteomics and biomarker development provide unparalleled opportunities for unraveling substantial injury-specific and patient-specific variability and refining disease characterization. The identification of novel, sensitive, objective tools, referred to as biomarkers, can revolutionize pathophysiological insights, enable targeted therapies and personalized approaches to clinical management. Approximately 10% of people can expect to have at least one epileptic seizure during a normal lifespan, and one third of these will develop epilepsy. In our study, we report most of the potential biomarkers of epilepsy to date, according to their magnitude in either diagnosis of epilepsy or estimation of the efficacy of antiepileptic treatment through reviewing the significance of each of: pathologic High-Frequency Oscillations pHFOs, brain inflammation, Blood Brain Barrier BBB dysfunction, Traumatic Brain Injury TBI and microRNAs as a potential biomarker of epilepsy. High-Frequency Oscillations as a Biomarker in EpilepsyPathological high-frequency oscillations (pHFOs) (HFOs, >80Hz) , identified in interictal EEG recordings seem to have the potential to line the range of epileptogenic tissue responsible for producing spontaneous seizures. interictal HFOs with spectral frequencies between 250 and 600 Hz, termed fast ripples (FR), are found in brain areas capable of generating spontaneous seizures, including dentate gyrus, hippocampus, subiculum and entorhinal cortex. Several retrospective studies that have followed up on patients after resective surgery found that removal of tissue generating interictal pathological HFOs correlated with a good outcome. Table 1.In conclusion, several studies with microelectrodes conclude that fast ripples are most specific for the epileptogenic zone, but in clinical studies with macroelectrodes it is useful to include both ripples and fast ripples in the evaluation of the potential epileptogenic region.Table 1. Blockade of IL 1b signaling leads to prevention of seizure generalization or delays stage 5 seizure occuru00acrence, respectively, and increases the threshold for afterdischarge induction.Inhibition of COX-2 during the latent phase, which follows status epilepticus, and is prodromal to spontaneous seizures onset, has given variable results. This phenomenon was also described following epileptifom activity induced by bicuculline in an in vitro guinea pig preparau00action. Two studies involving 245 human participants have shown that miR-106b-5p was significantly dysregulated in Epilepsy with one study claiming that it had the best diagnostic value epilepsy with 80.3 % sensitivity and 81.2 % specificity (Wang at el. 2015).ConclusionAmong all pHFOs, fast ripples seem to be more specific for the epileptogenic zone. Activation of the IL 1b system, COX-2 and mTOR are the major inflammatory pathways that many studies have shown their possible contribution to epileptogenesis. As for BBB dysfunction, ICAM 1, VCAM 1 and E-and P selectin studies have shown their close linkage to status epilepticus. So far miRNA 301a-3p and MiRNA-106b-5p have shown the best diagnostic efficacy for epilepsy with high sensitivity and specificity values.Although current evidences demonstrate that a definite biomarker of epilepsy is almost impossible, our review helped to get us closer to what might be a biomarker of epilepsy.

✏Identifying Potential Biomarkers of Posttraumatic Epilepsy Book Summary : Posttraumatic epilepsy (PTE) is a long-term negative consequence of traumatic brain injury (TBI) in which recurrent spontaneous seizures occur after the initial head injury. PTE develops over an undefined period where circuitry reorganization in the brain causes permanent hyperexcitability. Unfortunately, current existing antiepileptogenic drugs (AEDs) have all failed at treating PTE, and thus, there is a critical need to identify biomarkers of PTE to ultimately develop new therapeutic strategies. The pathophysiology by which trauma leads to spontaneous seizures is unknown and clinically relevant models of PTE are key to understanding the molecular and cellular mechanisms underlying the development of PTE. Current animal studies of PTE are limited and comprehensive in vivo electrophysiological approaches remain absent. In the present study, I aimed to identify optical and electrographic biomarkers of PTE with correlation to hippocampal histopathology at 14, 30, 60, and 90 days post injury (dpi). Here, adult male CD1 wildtype (WT) and aquaporin-4 knockout (AQP4 KO) mice were subjected to a moderate-severe TBI in the right frontal cortex using the well-established controlled cortical impact (CCI) injury model. Additionally, mice underwent optical coherence tomography (OCT) imaging, in vivo video-electroencephalographic (vEEG) recordings, and immunohistochemistry and Western blot analysis for the key epileptogenic astrocytic channels AQP4 and Kir4.1. The main findings from these studies are: 1) successful implementation of CCI-based PTE in mice with chronic vEEG generated, for the first time,17% and 27% of WT and AQP4 KO mice with PTE, respectively (the highest yield of PTE reported); 2) AQP4 KO mice had a greater incidence of spontaneous seizures and PTE compared with WT mice; 3) AQP4 KO mice had longer spontaneous seizure duration compared with WT mice; 4) EEG power patterns are different between mice with and without PTE; and 5) AQP4, but not Kir4.1, is significantly upregulated in the frontal cortex and hippocampus of mice with PTE. Collectively, these findings identified specific PTE EEG phenotypes that may be modulated by AQP4 and carry significant implications for epileptogenesis after TBI which may serve as the first steps to developing surrogate biomarkers for PTE.
